ePlasticMD is a referral and matching service that connects prospective patients with independent, board-certified plastic surgeons in our network. ePlasticMD does not provide medical care, does not employ the surgeons in our network, and is not a healthcare provider.
We use the information you provide to match you with a board-certified surgeon in our network, to communicate with you about your request, to process your $99 reservation credit, and to improve our services. We may also use your contact information to send you information about ePlasticMD or partner promotions, which you can opt out of at any time.
We share your information with the plastic surgeon(s) we match you with so they can contact you to schedule a consultation. We may also share information with service providers who help us operate our site (such as form-processing and email-delivery vendors) and payment processors. We do not sell your personal information to third parties for their own marketing purposes.
ePlasticMD is not a healthcare provider and is not a "covered entity" under HIPAA. Information you submit through our matching form is not part of a medical record. Once you are connected with a surgeon's office, any medical information you share directly with that practice is subject to that practice's own privacy practices and HIPAA obligations, not ours.
We retain your information for as long as reasonably necessary to provide our matching service and for legitimate business purposes, such as maintaining records of transactions. We use reasonable administrative and technical safeguards to protect your information, but no method of transmission or storage is 100% secure.
You may request access to, correction of, or deletion of your personal information by contacting us using the information below. You may also unsubscribe from marketing emails at any time using the unsubscribe link in those emails.
Our services are intended for individuals 18 years of age or older. We do not knowingly collect personal information from anyone under 18.
